• Peer-Run Respite Guest Recommendation Form

If you need assistance filling out this form, please feel free to call 508-751-9600
  • Hello, we are really glad you're here. We know reaching out during a crisis is often very hard.

We have made thoughtful updates to our model in 2026. If you've been a Guest with us before, it might feel a little different. The information below can help you determine if our program is right for you.

Kiva Centers’ respites remain fully peer-run and voluntary, grounded in a few core beliefs:

    • You are the expert in your own life.
    • Healing looks different for everyone, and a meaningful life is possible.
    • We support you to take the lead. We will share information, options, resources, emotional tools, or just listen without needing to try to "fix" anything, because we know that sometimes the pain or suffering we are going through cannot be easily resolved or made right.
    • Your rights, choices, and autonomy come first.
    • Your story belongs to you. You never have to explain, relive, or retell it to receive support.

    What's changed?

Kiva Centers is focused on being clear and transparent about what brings people to stay with us. As required by the Department of Mental Health (DMH), Kiva Centers has added a brief opening reflection document to the check-in process which names what you’re hoping for during your time here, whether that’s rest, new tools, support thinking through a challenge, or being met with care and compassion during a hard moment. We’ll write these intentions down together and revisit them at the end of your stay.

We’ve also strengthened a few safety practices, including confirming that everyone can respond in the event of a fire or other emergency, and discussing how you’d like us to support you if you begin to feel distressed, so the space remains supportive and safe for all.

  • Peer-Run Respite Guest Recommendation Form

  • Please note that our policy is to first reach out directly to the potential Guest. If we do not receive a response within 24 hours, we will follow up with a courtesy call to the Recommender to assist in establishing contact with the potential Guest.

  • Values Agreement

At Kiva Centers, we offer a space of warmth and understanding for Guests seeking solace and support. We embrace the values of self-determination, reciprocity, safety and respect, as we strive to foster a home where all people are valued.

As I complete this Recommendation form, I pause to consider my own readiness to contribute positively to our shared space. Am I in a mental state that aligns with the principles of respect and dignity for fellow Guests, staff, and the land we gather on? I recognize the gravity of this responsibility, and I commit to upholding these principles in my actions.

I acknowledge that if I am not able to uphold these values or compromises the sanctity of our shared environment, I may be asked to leave or no longer be eligible for peer-run respite support in the future.

  • Service Model:

Kiva Centers respites are not a locked setting. You’re welcome to bring your own clothes and shoes, and we do not monitor or restrict your movement during your stay. If at any point you feel that a more structured or supervised environment would better support your safety, that’s okay — and we’re here to help you think through options that may feel more supportive for you.

Kiva Centers respites are designed for a short-term stay, typically lasting 5-7 days. We are not designed for long-term support, but rather as a bridge to other resources or as a place of rest and sancutary.

We understand that during a crisis, it can feel natural to want the problem taken care of. Our program is designed to offer information, options, and steady support: to walk alongside you, rather than take over tasks or act as case managers. As part of my stay, I understand that I remain responsible for taking next steps using the tools, resources, or skills I gain here, and that some challenges may not have a clear or immediate resolution.

  • In this next session, we're going to ask you some questions about you, such as your age, race, and gender. These questions help us get the funding we need to continue running our homes and making our programs better.

Your personal information is kept stictly confidential. When we report on the data, we don't use any names or identifying information; we compile all the data into statistics.

You don't have to answer any questions you don't want to, and it won't change whether you can stay with us or not. It's totally up to you!
